How Life Changes Affect Medicare

Medicare is not a set-it-and-forget-it program. Certain life changes can affect when you can enroll, what you pay, and what options are available to you. Here is what you need to know.


Turning 65:


Your Initial Enrollment Period (IEP) is a 7-month window:


  • Starts 3 months before your birthday month

  • Includes your birthday month

  • Ends 3 months after your birthday month


If you sign up in the first 3 months, coverage generally starts on the first day of your birthday month. If you sign up later in the window, coverage starts the first of the month after you enroll. If you miss this window without qualifying coverage, you may face late enrollment penalties that last as long as you have Medicare.


Retiring or Losing Employer Coverage:


  • If you or your spouse loses job-based health coverage, you have an 8-month Special Enrollment Period to sign up for Part B without a penalty beginning the month after employment and/or coverage ends, whichever comes first

  • Important: COBRA, retiree coverage, VA coverage, and Marketplace coverage do NOT count as employer coverage for this purpose and do not extend your SEP. In addition, COBRA is secondary to Medicare, meaning enrolling in Part B is necessary to ensure your claims are covered.


Marriage or Divorce:


  • Changes in household income can affect your Income-Related Monthly Adjustment Amount (IRMAA) which is the extra amount some people pay on top of their Part B and Part D premiums

  • If your income has changed due to a life event and you disagree with your IRMAA, you can request a review through Social Security


Death of a Spouse:


  • May affect your Part B premium if it changes your household income and IRMAA level

  • May affect Part A eligibility if it was based on a spouse's work record


Receiving a Disability Determination:


  • If you receive Social Security Disability Insurance (SSDI), you will automatically get Medicare after 24 months of disability benefits

  • If you have ALS, Medicare begins the month your SSDI benefits start with no waiting period
